NY Dance Artistry Policy, Code of Conduct
● Class placement
Most of the time, classes are divided by age but the placement within the class is determined by the teachers. (After taking trial classes). Drop-in classes are not available.
● Trial Classes
The School offers two trial classes for $30 each. This fee can be deducted from your tuition for the same semester session. Trials must be booked in advance and are non-refundable once paid.
● School Term
Kids and Junior programs are divided into two terms. The first is from September to the end of January, the second
term is from the end of January to mid June. We have a separate Summer Program for July and August.
New students can register anytime if we have a spot available.
● New Student and Registration
Students register using our registration form. All applications are considered in the order received.
New students will pay a $50 registration fee for the first time, $25 for additional siblings. Returning students will pay a $20 registration fee.
If students are not continuing to take classes, students should inform the school. If students are not taking classes for more than three months, a $25 re-registration fee will apply when coming back to the school.
● Payments
All payments are final and non-refundable. Tuition must be paid in the first week of the session. Once the payment is confirmed, the application will be officially completed. Due to holiday schedules, the number of classes per month may vary.
● Cancellation
If the class is unavoidably canceled due to weather (natural disasters such as heavy snow, hail, hurricanes, storms, extreme weather) or emergencies (terrorism, fire, public transportation stopped), there will be no makeup lessons. In addition, refunds are not possible.
Classes will be canceled if public schools in New York City are closed for the reasons mentioned above.
In case of cancellation due to the above reasons, we will contact you by email.
If an emergency is issued in an area other than New York City, students living in that area should be voluntarily absent for safety.
If your class is canceled for any other reason, there may be make-up classes. Please follow the school's announcements.
A class may be subject to cancellation dependent upon student enrollment. In such cases, a School Coordinator will contact all students in advance to coordinate alternative class and tuition options.
● Attendance
Good attendance is imperative to a dancer's progress. Students who are injured are required to watch class, although students who are ill and contagious should stay home. If a student is injured, the student can observe the class in the studio.
If there are unavoidable circumstances (moving, bereavement of relatives), or if there is a doctor's stop from the doctor for health reasons and you cannot attend the lesson for 30 days or more, the tuition fee will be credited for the next year but not refundable.
In that case, please submit a document from an appropriate organization or doctor. This credit is applicable towards future school classes only. Not available for recitals and special events including the summer program.
● Missing Class
Please Email us before the class starts if a student will be absent.
Missed classes may be made up by appointment only within two weeks.
Students may make up an absence by either attending another section of their level or one level below. Make-up classes might not be permitted due to the space constraints in each class section. Students can take an online class for makeup.
● Dress Code
Girls are required to wear pink footed tights, ballet leotards, and pink ballet slippers. No tutus, Short skirt is allowed for Kids class. No skirt for Juniors. Pointe shoes upon teacher permission.
Males - white tank leotard or white tee shirt with black convertible tights, socks, and ballet slippers
Hair should be neatly put in a bun or pulled back for class. No jewelry except for stud earrings.
● INJURY OR LOSS OF PROPERTY
The risk of injury exists in any physical activity. Please consider this before enrolling your child.
NY Dance Artistry - Hariyama Ballet cannot be held responsible for personal injury or loss of property. Personal possessions should not be left in dressing rooms or restrooms under any circumstances.
● Photos and/or Videos
In registering your child, we have your permission to use class or performance photos for the marketing of NY Dance Artistry - Hariyama Ballet. If you object, please submit your objection at the time of registration. Parents may only take photos or videos on observation days with the approval of each individual instructor.
● Performance
We have one or more performances during the year. All students will be participating in the performances. Participation in the Spring Performance is mandatory for all classes. It’s a part of the curriculum for the NY Dance Artistry - Hariyama Ballet. Performance fees will be separately charged and the cost will be depending on ages and numbers of pieces the students perform. We will announce details before the performance rehearsal begins. Normally the performance fee is between $150 - $300.
Ballet costume rental price may vary by age and type of costume. Rental fee is usually between $30 to $80 per costume. Other classes are depending on the piece and teacher's choice.
● Responsibilities of Parents & Guardians
NO Kids Class Students are allowed in the studios without a teacher present at any time. Please fold ALL strollers and leave them in the stroller parking area. DO NOT block the elevator, stairwell, or hallway and help us to keep our building safe and clear! Parents or guardians are not permitted to observe class, take a photo or film unless the teacher gives permission.

●Change or Code of Conduct
The contents of the Code of Conduct are subject to change, addition or update. Changes, additions, and updates will take effect from the time they are notified by email or posted on our school website.
LIABILITY AGREEMENTS

I understand that I am responsible for the supervision of my child before and after class.

I agree to pick my child up promptly after class and to advise Hariyama Ballet / NYDA staff if someone other than a parent or guardian will be picking him/her/them up.

I am aware that dance education at times requires hands-on training, during which instructors will gently touch a child’s body to explain proper alignment, positioning or engagement of their muscles.

I understand that personal possessions should not be left in dressing rooms or restrooms under any circumstances and release Hariyama Ballet / NYDA from all liability with respect to any lost property

I understand that young children who may need assistance in the restroom will be assisted by an instructor or administrative staff member. I authorize Hariyama Ballet / NYDA personnel to assist my child in using the restroom if needed.

I hereby give my permission to Hariyama Ballet / NYDA to photograph, film, videotape, and/or make sound recordings of my child, to quote or publish statements of my child, and to use such photographs, films, videotapes, sound recordings, and/or other statements in educational and promotional/advertising materials for Hariyama Ballet / NYDA and for other purposes specified below. I understand that my child may be identified in any photographs, news stories, or publications that Hariyama Ballet / NYDA considers appropriate for release to magazines, newspapers, its website, social media, and/or other publications. I further understand that any such photographs, films, videotapes, sound recordings, and/or written works are the property of Hariyama Ballet / NYDA and that neither my child nor I am entitled to any compensation for or rights in these materials. I release Hariyama Ballet / NYDA from all liability with respect to the matters covered by this release.

I understand that Hariyama Ballet / NYDA does not prohibit parents and guardians from taking their own photographs and making their own audio visual and sound recordings of their children on specified class observation days or at recitals. I understand that Hariyama Ballet / NYDA has no control over the use of these materials by parents and guardians or others. I release Hariyama Ballet / NYDA from all liability with respect to the use of these materials created and distributed by parents and guardians or others.

I understand that dancing is an enjoyable form of exercise, which provides benefits such as good posture, flexibility, strength, and agility. I also understand that, as with any physical activity, there naturally exists a risk of injury. By signing this registration form, I agree that neither Hariyama Ballet /NY Dance Artistry (NYDA), nor its agents or employees will be held responsible in the event of an injury to me or my child or ward.

I understand and agree that participating in any dance lesson or class, workshop, rehearsal or performance, there, along with physical injury, is a possibility of mental stress. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, mental or otherwise, which might occur to me (or my child) during any of Hariyama Ballet / NY Dance Artistry classes, lessons, rehearsals, performances, or activities. I also exempt, release, and indemnify Hariyama Ballet / NYDA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Hariyama Ballet / NYDA. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Hariyama Ballet / NYDA, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical and mental limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

I certify that I. or my child has no condition that, in the exercise of reasonable prudence, would prohibit participation in activities at Hariyama Ballet / NYDA. I agree to inform instructors of any physical or developmental needs that may require special attention. I assume all ordinary risks for my child’s participation in dance classes and hereby release Hariyama Ballet / NYDA and its agents and employees from all liability for any injury, damage, or loss suffered in connection with the use of Hariyama Ballet / NYDA’s facilities and equipment.

Should a student display action or behavior that disrupts a class, or antagonizes others in class, the teacher at their discretion may ask the student to leave the class or cease participation if Guardian is not available. The student will not be reimbursed for the class fee. Repeated offenses will subject the student to expulsion from Hariyama Ballet / NYDA with no financial reimbursement.

In case of an emergency, when neither parent or guardian listed on the registration form can be reached, I authorize Hariyama Ballet / NYDA personnel, if necessary, to contact and secure medical attention for my child. I certify that my child has health insurance.
